The Business and Ethics of the Brain Fitness Boom — Part 1: The Business

The recent dis­cov­ery that expe­ri­ence can change brain struc­ture and func­tion at any age has sparked numer­ous health, edu­ca­tion, and pro­duc­tiv­ity appli­ca­tions whose value and lim­i­ta­tions we are only start­ing to grasp.

Brain fit­ness has quickly become a main­stream aspi­ra­tion among baby boomers and elders, pri­mar­ily in North Amer­ica. It has fueled a grow­ing inter­est in brain fit­ness classes, brain fit­ness cen­ters, and brain fit­ness pro­grams, along with atten­dant oppor­tu­ni­ties and chal­lenges. An increas­ing num­ber of adults want use­ful tools to pro­tect cog­ni­tive health and performance—not nec­es­sar­ily to reverse aging—and what they are find­ing is an expand­ing and noisy mar­ket­place where they (and also pro­fes­sion­als) need to care­fully eval­u­ate their own needs and the avail­able options (Fer­nan­dez and Gold­berg, 2009). A Brain Store to Find the Right Brain Fitness Products

Baby­boomers, wor­ried about demen­tia, are look­ing for ways to keep their brain active. Board games and brain fit­ness soft­ware pro­grams are part of the answer. How­ever it is not always easy to find and com­pare these prod­ucts. This arti­cle shows how Mar­ble: The Brain Store has devel­oped a very inter­ac­tive retail expe­ri­ence to help con­sumers find the prod­ucts that are right for them.

Another victim of the BBC/Nature “brain training” experiment

Have you read the cover story of the New Sci­en­tist this week: Men­tal mus­cle: six ways to boost your brain?

The arti­cle, which includes good infor­ma­tion on brain food, the value of med­i­ta­tion, etc., starts by say­ing that: “Brain train­ing doesn’t work, but there are lots of other ways to give your grey mat­ter a quick boost.” Fur­ther in the arti­cle you can read “… brain train­ing soft­ware has now been con­signed to the shelf of tech­nolo­gies that failed to live up to expectations.”

Such claims are based on the one study widely pub­li­cized ear­lier this year: the BBC “brain train­ing” exper­i­ment, pub­lished by Owen et al. (2010) in Nature.

What hap­pened to the sci­en­tific rigor asso­ci­ated with the New Scientist?

New Report Finds A Brain Health Revolution in the Making, Driven by Digital Technology and Neuroplasticity Research

2010MarketReportIn spite of the recent eco­nomic down­turn, rev­enues for dig­i­tal tech­nolo­gies to assess, enhance and treat cog­ni­tion, or dig­i­tal brain health and fit­ness tools, grew 35% in 2009. “The con­ver­gence of demo­graphic and pol­icy trends with cog­ni­tive neu­ro­science dis­cov­er­ies and tech­no­log­i­cal inno­va­tion is giv­ing birth to a nascent mar­ket­place that can fun­da­men­tally trans­form what brain health is, how it is mea­sured, and how it is done,” says Alvaro Fer­nan­dez, mem­ber of the World Eco­nomic Forum’s Coun­cil on the Aging Soci­ety and Editor-in-Chief of the report. “This ground­break­ing report can help pio­neers shape the emerg­ing toolkit to ben­e­fit an aging soci­ety that increas­ingly seeks new ways to enhance cog­ni­tive func­tion­al­ity and men­tal well­ness across the lifespan.”

As the brain is thrust into the cen­ter of the health­care ecosys­tem, inno­v­a­tive cog­ni­tive health and brain fit­ness appli­ca­tions will play an increas­ingly impor­tant role in defin­ing neu­ro­cen­tric health,” adds Jake Duna­gan, Research Direc­tor at the Insti­tute For The Future.

Report: Trans­form­ing Brain Health with Dig­i­tal Tools to Assess, Enhance and Treat Cog­ni­tion across the Lifes­pan: The State of the Brain Fit­ness Mar­ket 2010.

A major­ity among the 1,900+ decision-makers and early-adopters sur­veyed said they trusted the effec­tive­ness of non-invasive options above inva­sive options to enhance crit­i­cal brain func­tion­al­ity. Pro­fes­sional and intel­lec­tual chal­lenges were rated very effec­tive by 61% of respon­dents, aer­o­bic exer­cise and read­ing books by 42%, med­i­ta­tion by 38%, com­put­er­ized brain train­ing by 26%, tak­ing pre­scrip­tion drugs by 13%, tak­ing sup­ple­ments by 12%, and self-medicating with drugs by 1%.

These are among the key find­ings of a 207-page mar­ket report released today by Sharp­Brains and pre­pared in col­lab­o­ra­tion with 24 lead­ing sci­en­tists and 10 inno­v­a­tive orga­ni­za­tions — the most com­pre­hen­sive such research study done to ana­lyze emerg­ing research, tech­nolo­gies and marketplace.

We must do for brain health in the 21st cen­tury what we largely accom­plished in car­dio­vas­cu­lar health in the past cen­tury. It’s time to take sci­en­tific insights out of the lab and to iden­tify prac­ti­cal appli­ca­tions, mak­ing the main­te­nance of good brain fit­ness a pub­lic health pri­or­ity,” indi­cates William Reich­man, MD, Pres­i­dent and CEO of Baycrest.

Education AND Lifelong Cognitive Activities build Cognitive Reserve and Delay Memory Loss

In a recently pub­lished sci­en­tific study (see Hall C, et al “Cog­ni­tive activ­i­ties delay onset of mem­ory decline in per­sons who develop demen­tia” Neu­rol­ogy 2009; 73: 356–361), Hall and col­leagues exam­ined how edu­ca­tion and stim­u­lat­ing activ­i­ties may inter­act to con­tribute to cog­ni­tive reserve. The study involved 488 ini­tially healthy peo­ple, aver­age age 79, who brain teasers job interviewenrolled in the Bronx Aging Study between 1980 and 1983. These indi­vid­u­als were fol­lowed for 5 years with assess­ments every 12 to 18 months (start­ing in 1980). At the start of the study, all par­tic­i­pants were asked how many cog­ni­tive activ­i­ties (read­ing, writ­ing, cross­word puz­zles, board or card games, group dis­cus­sions, or play­ing music) they par­tic­i­pated in and for how many days a week. Researchers were able to eval­u­ate the impact of self-reported par­tic­i­pa­tion these activ­i­ties on the onset of accel­er­ated mem­ory decline in 101 indi­vid­u­als who devel­oped demen­tia dur­ing the study.

Results showed that for every “activ­ity day” (par­tic­i­pa­tion in one activ­ity for one day a week) the sub­jects engaged in, they delayed for about two months the onset of rapid mem­ory loss asso­ci­ated with demen­tia. Inter­est­ingly, the pos­i­tive effect of brain-stimulating activ­i­ties in this study appeared to be inde­pen­dent of a person’s level of education.

This is great news as it sug­gests that it is never too late to try to build up brain reserve. The more brain stim­u­lat­ing activ­i­ties one does and the more often, the bet­ter for a stronger cog­ni­tive reserve.

The cog­ni­tive reserve hypoth­e­sis sug­gests that indi­vid­u­als with more cog­ni­tive reserve can expe­ri­ence more Alzheimer’s dis­ease pathol­ogy in the brain (more plaques and tan­gles) with­out devel­op­ing Alzheimer’s dis­ease symptoms.

How does that work? Sci­en­tists are not sure but two pos­si­bil­i­ties are con­sid­ered.
1. One is that more cog­ni­tive reserve means more brain reserve, that is more neu­rons and con­nec­tions between neu­rons.
2. Another pos­si­bil­ity is that more cog­ni­tive reserve means more com­pen­satory processes (see my pre­vi­ous post “Edu­ca­tion builds Cog­ni­tive Reserve for Alzheimers Dis­ease Pro­tec­tion” for more details.)

Now, one may won­der about the dif­fer­ence types of men­tal stim­u­la­tion avail­able, includ­ing not only puz­zles and such, but struc­tured activ­i­ties such as brain fit­ness soft­ware and med­i­ta­tion. Do we exer­cise our brain every time we think about some­thing? What can one do to exer­cise one’s brain in ways that enhance capac­ity? Does aer­o­bic fit­ness train­ing also exer­cise one’s brain? What types of method­olo­gies and prod­ucts are avail­able? Do they “work”? Are all the same?

Prepar­ing Soci­ety for the Cog­ni­tive Age

Ground­break­ing cog­ni­tive neu­ro­science research has occurred over the last 20 years — with­out par­al­lel growth of con­sumer aware­ness and appro­pri­ate pro­fes­sional dis­sem­i­na­tion. “Cog­ni­tion” remains an elu­sive con­cept with unclear impli­ca­tions out­side the research community.

Ear­lier this year, I pre­sented a talk to health care pro­fes­sion­als at the New York Acad­emy of Med­i­cine, titled “Brain Fit­ness Soft­ware: Help­ing Con­sumers Sep­a­rate Hope from Hype”. I explained what com­put­er­ized cog­ni­tive assess­ment and train­ing tools can do (assess/enhance spe­cific cog­ni­tive func­tions), what they can­not do (reduce one’s “brain age”) and the cur­rent uncer­tain­ties about what they can do (i.e., delay Alzheimer’s symp­toms). At the same sym­po­sium, Dr. Gary Kennedy, Direc­tor of Geri­atric Psy­chi­a­try at Mon­te­fiore Med­ical Cen­ter, pro­vided guid­ance on why and how to screen for exec­u­tive func­tion deficits in the con­text of dementia.

I could per­ceive two emerg­ing trends at the event: 1) “Aug­ment­ing Cog­ni­tion” research is most com­monly framed as a health­care, often phar­ma­co­log­i­cal topic, with the tra­di­tional cog­ni­tive bias in med­i­cine of focus­ing on detec­tion and treat­ment of dis­ease, 2) In addi­tion, there is a grow­ing inter­est in non-invasive enhance­ment options and over­all lifestyle issues. Research find­ings in Aug­ment­ing Cog­ni­tion are only just begin­ning to reach the main­stream mar­ket­place, mostly through health­care chan­nels. The oppor­tu­nity is immense, but we will need to ensure the mar­ket­place matures in a ratio­nal and sus­tain­able man­ner, both through health­care and non-healthcare channels.

In Jan­u­ary 2009, we polled the 21,000 sub­scribers of Sharp­Brains’ mar­ket research eNewslet­ter to iden­tify atti­tudes and behav­iors towards the “brain fit­ness” field (a term we chose in 2006 based on a num­ber of con­sumer sur­veys and focus groups to con­nect with a wider audi­ence). Over 2,000 decision-makers and early adopters responded to the survey.

One of the key ques­tions we asked was, “What is the most impor­tant prob­lem you see in the brain fit­ness field and how do you think it can be solved?”. Some exam­ples of the sur­vey free text answers are quoted here, together with my suggestions.

Most impor­tant prob­lems in the brain fit­ness field

Pub­lic aware­ness (39%): “To get peo­ple to under­stand that hered­ity alone does not decide brain func­tion­ing”. We need to ramp up efforts to build pub­lic aware­ness and enthu­si­asm about brain research, includ­ing estab­lish­ing clear links to daily liv­ing. We can col­lab­o­rate with ini­tia­tives such as the Dana Foundation’s Brain Aware­ness Week and use the recent “Neu­ro­science Core Con­cepts” mate­ri­als devel­oped by the Soci­ety for Neu­ro­science to give talks at schools, libraries and workplaces.

You may be read­ing all about brain fit­ness and brain train­ing. It seems every week brings a new bar­rage of arti­cles and stud­ies which often con­tra­dict what you read the month before: Does Gingko Biloba help delay Alzheimer’s Dis­ease?
