In the past two days, The New York Times has published two excellent articles on brain and cognitive fitness. Despite appearing in separate sections (technology and editorial), the two have more in common than immediately meets the eye. Both raise key questions that politicians, health policy makers, business leaders, educators and consumers should pay attention to.

Recapping from last months article (see Stress and Neural Wreckage: Part of the Brain Plasticity Puzzle)...our bodies are a complex balancing act between systems working full time to keep us alive and well. Any change which threatens this balance can be referred to as stress. Cortisol, a key component of the stress response, does an excellent job of allowing us to adapt to most stressors which last more than a couple of minutes. However, having to endure a high stressor for longer than about 30 minutes to an hour negatively impacts the brain in various ways.

“My brain is…fried, toast, frazzled, burnt out.” How many times have you said or heard one version or another of these statements. Most of us think we are being figurative when we utter such phrases, but research shows that the biological consequences of sustained high levels of stress may have us being more accurate than we would like to think.

Crash Course on Stress

Our bodies are a complex balancing act between systems working full time to keep us alive and well. This balancing act is constantly adapting to the myriad of changes occurring every second within ourselves and our environments. When it gets dark our pupils dilate, when we get hot we sweat, when we smell food we salivate, and so forth. This constant balancing act maintains a range of stability in the body via change; and is often referred to as allostasis. Any change which threatens this balance can be referred to as allostatic load or stress.

Allostatic load/stress is part of being alive. For example just by getting up in the morning, we all experience a very important need to increase our heart rate and blood pressure in order to feed our newly elevated brain. Although usually manageable, this is a change which the body needs to adapt to and, by our definition, a stressor.

Brain Evolution and Why it is Meaningful Today to Improve Our Brain Health

You may feel overwhelmed by the stream of seemingly contradictory suggestions regarding the best way to maintain mental clarity as you age. Based on an analysis of seminal factors in the development of modern brain anatomy, I believe it is possible to make some very compelling recommendations for growing big brains, enhancing their function, and making them resistant to the aging process. These may be loosely categorized as factors pertaining to the mental or physical attributes of the brain. Although they are not truly independent entities, such a conceptualization provides a basis for the generation of brain healthy prescriptions. Diet, physical exercise, and stress reduction enhance neuronal resilience. Sleep and mental stimulation are vital for cognitive ability, learning, and memory.

Diet: Follow a modern shore-based/marine diet including seafood in its most general sense, non-starchy vegetables of all colors, berries, and eggs. Other sources of lean protein containing long-chain omega 3 fatty acids such as free range beef, chicken, bison, or elk are nutritious alternatives.

Physical exercise (Think ‘fight or flight’ activity.): Include all types. Aerobic activities such as swimming, bicycling, walking, or hiking for promotion of vascular health and weight control; resistance training for promotion of neurotrophic factors, naturally occurring compounds that make brain cells more resistant to aging, such as IGF-1 (Insulin-like growth factor-1) and BDNF (Brain-derived neurotrophic factor); and balance, coordination, and agility training such as ping-pong, balance beam, trampoline, and jumping rope to enhance cognitive speed and motor skills.

Stress Control: From an evolutionary perspective, stressors (such as meeting a cave bear) and intense physical activity (running or fighting) were brief in duration and usually occurred together. Modern stressors (psychological or emotional stress) tend to be unremitting and are generally uncoupled from the physical (fight or flight) component, meaning stress develops without any associated physical activity. Such intense physical pursuits are now called exercise. Not surprisingly, exercise is a perfect physiologic antidote for stress due to its beneficial impact on cortisol (the ‘stress’ hormone) and blood pressure and should be incorporated into any program of stress reduction.

Adequate sleep: The body needs rest, but the brain requires sleep. Acute or chronic sleep deprivation causes devastating short and long-term consequences to brain anatomy (synaptic loss) and function (memory and learning difficulties). Off-line information processing and memory consolidation are additional sleep-related benefits.

Mental stimulation: Brain-training, a cognitively challenging lifestyle, novelty, and socialization are vital for the promotion of neuronal plasticity and neurogenesis (the formation of new nerve cells and neuronal connections), the enhancement of specific brain functions such as memory, and the development of cognitive reserve –additional mental processing potential that may be brought online when needed.

The combination of these recommendations, each of which was instrumental in the transformation from primitive to modern nervous systems, provides a template for the most logical approach for enhancing mental function and resisting neurodegeneration as we travel through life.

Over the last year we have interviewed a number of leading brain health and fitness scientists and practitioners worldwide to learn about their research and thoughts, and have news to report.

What can we say today that we couldn't have said only 10 years ago? That what neuroscience pioneer Santiago Ramon ySantiago Ramon y Cajal Cajal claimed in the XX century, "Every man can, if he so desires, become the sculptor his own brain", may well become reality in the XXI. And influence Education, Health, Training, and Gaming in the process.

We have only scratched the surface of what science-based structured cognitive (i.e., mental) exercise can do for brain health and productivity. We are now witnessing the birth of a new industry that crosses traditional sector boundaries and that may help us understand, assess and train our brains, harnessing the growing research about neurogenesis (the creation of new neurons), neuroplasticity (the ability of the brain to rewire itself through experience), cognitive training and emotional regulation.

Let's now debunk 10 myths, still too prevalent, that may prevent us from seeing the full potential of this emerging field:

Myth 1: It’s all in our genes.

Reality: A big component of our lifelong brain health and development depends on what we do with our brains. Our own actions, not only our genes, influence our lives to a large extent. Genes predispose us, not determine our fates.

• “Individuals who lead mentally stimulating lives, through education, occupation and leisure activities, have reduced risk of developing Alzheimer’s. Studies suggest that they have 35-40% less risk of manifesting the disease” - Dr. Yaakov Stern, Division Leader of the Cognitive Neuroscience Division of the Sergievsky Center at Columbia University.

Myth 2: The field of Cognitive/ Brain Fitness is too new to be credible.

Reality: The field rests on solid foundations dating back more decades --- what is new is the number and range of tools that are now starting to be available for healthy individuals.

An anonymous reader of Andrew Sullivan's blog writes a superb comment, reproduced here:

"One thing Watson and others forget is that the brain is highly malleable based on environment. Although he is the father of DNA he knows very little about neuroplasticity and neurogenesis. Previously it was thought that the human brain was 'hardwired' after a certain age. This is not true. Not only is not true, but the human mind is capable of adaptation but actual neuron growth even late in life. Ten years ago this was thought impossible.

Neurogenesis and neuroplasticity proves that a nurturing social and family setting shifts IQ, perspective, and emotional IQ. The so-called bell curve isn't genetic. Oppressed Tibetans and Chinese ethnic minorities -whose test scores soar in the United States and Canada- are 20-30 points lower in their homeland. That 20-30 points deficit is in the same range of a lot of groups that are attacked or threatened (Muslims in France, Christians in Nigeria, Blacks in America). Conversely when oppressed groups are removed from their environment their IQ, emotional health returns to a normal rate, thus proving that is NOT genetic.

It is plastic, shifting and based upon the environment.

Best of Brain, Scientific American

The Dana Foundation kindly sent us a copy of the great book Best of the Brain from Scientific American, a collection of 21 superb articles published previously in Scientific American magazine. A very nicely edited and illustrated book, this is a must for anyone who enjoys learning about the brain and speculating about what the future will bring us.

Some essays, like the ones by Eric Kandel (The New Science of Mind), Fred Gage (Brain, Repair Yourself), Carl Zimmer (The Neurobiology of the Self) and that by Steven Hollon, Michael Thase and John Markowitz (Treating Depression: Pills or Talk), are both intellectual feasts and very relevant to brain fitness. And finally starting to percolate into mainstream consciousness.

Let me quote some quotes and reflections as I was reading the book a couple of days ago, in the courtyard of a beautiful French cafe in Berkeley:

1)  On Brain Plasticity (the ability of the brain to rewire itself), Fred Gage says: "Within the past 5 years, however, neuroscientists have discovered that the brain does indeed change throughout life-...The new cells and connections that we and others have documented may provide the extra capacity the brain requires for the variety of challenges that individuals face throughout life. Such plasticity offers a possible mechanism through which the brain might be induced to repair itself after injury or disease. It might even open the prospect of enhancing an already healthy brain's power to think and ability to feel"  

2)  and How Experience affects Brain Structure: Under the section title "A Brain Workout", Fred Gage says "One of the mot striking aspects of neurogenesis (Note: the creation of new neurons) is that experience can regulate the rate of cell division, the survival of newborn neurons and their ability to integrate into the existing neural circuits...The best way to augment brain function might not involve drugs or cell implants but lifestyle changes."

Given the growing number of articles in the popular press mentioning words such as "neuroplasticity", "fMRI" and "cognitive reserve", let's review some key findings, concepts and terms.

First, a prescient quote by Spanish neuroscientist Santiago Ramon y Cajal (1852-1934): "Every man can, if he so desires, become the sculptor his own brain".

fmri.jpgThanks to new neuroimaging techniques, regarded “as important for neuroscience as telescopes were for astronomy”, neuroscientists and cognitive psychologists have been finding that the brain has a number of "core capacities" and "mental muscles" that can be exercised through novelty, variety and practice, and that exercising our brain can influence the generation of new neurons and their connections. Brain exercise is being recognized, therefore, as a critical pillar of brain health, together with nutrition, physical exercise and stress management.

Previous beliefs about our brain and how it works have been proven false. Some beliefs that have been debunked include claims that adult brains can not create new neurons (shown to be false by Berkeley scientists Marian Diamond and Mark Rosenzweig, and Salk Institute’s Fred Gage), notions that working memory has a maximum limit of 6 or 7 items (debunked by Karolinska Institute’s Torkel Klingberg), and assumptions that the brain’s basic processes can not be reorganized by repeated practice (UCSF’s Drs. Paula Tallal and Michael Merzenich). The "mental muscles" we can train include attention, stress and emotional management, memory, visual/ spatial, auditory processes and language, motor coordination and executive functions like planning and problem-solving.

Mental stimulation is important if done in the right supportive and engaging environment. Stanford’s Robert Sapolsky has proven that chronic stress and cortical inhibition, which may be aggravated due to imposed mental stimulation, may prove counterproductive. Having the right motivation is essential.

A surprising and promising area of scientific inquiry is Mindfulness-Based Stress Reduction (MBSR). An increasing number of neuroscientists (such as University of Wisconsin-Madison’s Richard Davidson) are investigating the ability of trained meditators to develop and sustain attention and visualizations and to work positively with powerful emotional states and stress through the directed mental processes of meditation practices.
