(Please note that this is my personal take at the discussions that took place in Dubai as part of the Global Agenda Council on the Challenges of Gerontology put together by the World Economic Forum, and builds on the work of my colleagues, but it does not represent a formal document or statement of position. Simply put, we would like to engage your brain in defining the challenges and outlining/ executing the solutions).

Context: The Challenges of the Aging Society

The world is aging. This is occurring in two ways: through shifts in the age structure that will eventually lead to many more people reaching older ages than ever before, and through continued success in extending life. Less than 100 years ago, life expectancy was between 30 to 40 years. Today, close to 800 million citizens are 60 and over.

And aging in healthier ways. Aging has incorrectly been associated with decline and decay, when in fact many people live healthy into older ages. There has been a synchronous extension in life expectancy and quality of life - the average 65-year-old today is much healthier, physically and mentally, than the average 50-year-old of 100-150 years ago - when most existing institutions were envisioned and created.

Healthy life can be further extended with existing knowledge. The fact is the onset and progression of fatal and disabling diseases, disorders, and disability can be postponed using well-researched basic measures of public health, environmental and behavioural changes, and medical technology interventions. The same methods may be used to improve or maintain mental and physical functioning.

Our healthcare and retirement systems are on bankruptcy track - their premises are outdated. Existing institutions, policies and attitudes do not reflect the points outlined above, having been developed for a society that no longer exists. We need to get on the right track: Continue Reading »

Just saw a very interesting press release regarding computer-based neurocognitive assessments - a critical part of the brain fitness puzzle. How long will it take before consumers can have access to a reliable and credible annual "mental check-up"/ cognitive baseline?

HeadMinder Cognitive Stability Index: Computerized Neurocognitive ... (Press release)

The August issue of Venture Capital Journal brings a very good piece on the emerging brain fitness software  (also called "neurosoftware") category: 

-- Mind Games (subscription required)

-- Dakim, Lumos Labs, Posit Science and other “brain fitness” startups are starting to gain mind share — and capital — from venture firms.

The reporter and I spoke as Lumos Labs received its $3m round, and we discussed other fundable start-ups, featuring CogniFit. Which, as mentioned over the weekend, just raised $5m.

What about your brain fitness?

Two recent announcements bring out how the assessment of cognitive abilities, or brain functions, is increasingly being done thanks to new computerized options:

1) Last week, OptumHealth announced an exclusive 3-year agreement (estimated at $18m) with the Australian company Brain Resource. OptumHealth will be embedding the Brain Resource platform into their overall Behavioral Solutions program.

- OptumHealth Behavioral Solutions will work with Brain Resource to provide clinicians with a Web-based assessment that measures general cognition (how people process information) and social cognition (how people manage their emotions). This 40-minute assessment is based on well-known and validated tests of memory, attention, executive function, and response speed, and mood, social skills and emotional resilience.

CNN: Aging boomers fuel 'brain fitness' explosion: An excellent article via Associated Press exploring why the brain fitness market passed a tipping point in 2007 and predicting future trends building on our market report.

Brain Age: Great Game, Wrong Concept: One reason why we believe the field will keep growing is because we are seeing more tools available than ever before to assess and train a variety of cognitive skills. A few colleagues referred me over the weekend to a very nice article at business publication Portfolio.

While the article does an excellent job at introducing the reader to the concept and promise of computerized cognitive assessments, it also contributes to the mythology of "Brain Age". MRI scan neuroimaging

Let's first take a look at the article How Smart Are You: The business of assessing cognition and memory is moving from testing brain-impaired patients to assessing healthy peoples' brains online.

A couple of quotes:

- "Cognitive Drug Research is one a handful of businesses, most of them outside of the U.S., that work with pharmaceutical companies to test how new drugs for everything from nicotine addiction to Alzheimer’s disease affect the mind’s ability to remember things, make decisions, and analyze information."

- "Cognitive tests have been around for a century as examinations taken with paper and pencil. In the 1970s and '80s the tests shifted to computers, Cognitive Drug Research founder Keith Wesnes says.

So far, so good. In fact, one of the key highlights from the market report we released in March was that "Large-scale, fully-automated cognitive assessments are being used in a growing number of clinical trials. This opens the way for the development of inexpensive consumer-facing, baseline cognitive assessments." And we profiled a few leading companies in the space: Brain Resource Company, Cognitive Drug Research, CNS Vital Signs and CogState.

Now, the article is accompanied by a 5-7 minute quick test that promises to give us our "Brain Age". And this doesn't come from Nintendo, but from Cognitive Drug Research, a respected science-based company.

I wrote this article for the March/ April edition of the publication Aging Today, published by the American Society on Aging.

In recent years, most professionals in aging have become aware of the growing scientific evidence showing that human brains retain the ability to generate neurons and change over a lifetime, discoveries that have broken the scientific paradigm prevalent during the 20th century. Furthermore, neuroimaging and cognitive training studies are showing how well-directed exercise presents people major opportunities for healthy brain aging.

How can people use emerging technologies to keep their brains healthy and productive as long as possible? An emerging market for brain health-- $225 million market in 2007, in the United States alone, of which consumers account for $80 million--is trying to address that question in a way that complements other important more traditional pillars (and multi-billion industries) of brain health, such as physical exercise, balanced diet, stress management (stress has been shown to actually kill neurons and reduce the rate of creation of new ones) and overall mental stimulation and lifelong learning.

2007 AN ACTIVE YEAR

A series of important events took place in 2007, a seminal year for the brain health field, beginning in January when many mainstream media publications, such as Time Magazine and CBS News, started to publish major stories on neuroplasticity and brain exercise. This media coverage followed the publication of the long-awaited results from national clinical trials showing that significant percentages of the participants age 65 and older who trained for five weeks improved their memory, reasoning and information-processing speed. Findings from the Advanced Cognitive Training for Independent and Vital Elderly (ACTIVE) Study appeared in the Journal of the American Medical Association (Dec. 20, 2006) and revealed that even after five years, participants in the ACTIVE computer-based program showed less of a decline in information-processing skills than those in a control group that received no cognitive training.

Today I had a great conversation with Martin Buschkuehl, one of the U Michigan researchers involved in the cognitive training study that has received much media attention since early last week, when the study was published at the Proceedings of the National Academy of Sciences.

I will publish the interview notes next week. For the moment, let me paraphrase his answer to the question: "Why are computerized programs like the one you used fundamentally different from, say, simply doing many crossword puzzles?".
