Education AND Lifelong Cognitive Activities build Cognitive Reserve and Delay Memory Loss

In a recently published scientific study (see Hall C, et al “Cognitive activities delay onset of memory decline in persons who develop dementia” Neurology 2009; 73: 356-361), Hall and colleagues examined how education and stimulating activities may interact to contribute to cognitive reserve. The study involved 488 initially healthy people, average age 79, who brain teasers job interviewenrolled in the Bronx Aging Study between 1980 and 1983. These individuals were followed for 5 years with assessments every 12 to 18 months (starting in 1980). At the start of the study, all participants were asked how many cognitive activities (reading, writing, crossword puzzles, board or card games, group discussions, or playing music) they participated in and for how many days a week. Researchers were able to evaluate the impact of self-reported participation these activities on the onset of accelerated memory decline in 101 individuals who developed dementia during the study.

Results showed that for every “activity day” (participation in one activity for one day a week) the subjects engaged in, they delayed for about two months the onset of rapid memory loss associated with dementia. Interestingly, the positive effect of brain-stimulating activities in this study appeared to be independent of a person’s level of education.

This is great news as it suggests that it is never too late to try to build up brain reserve. The more brain stimulating activities one does and the more often, the better for a stronger cognitive reserve.

The cognitive reserve hypothesis suggests that individuals with more cognitive reserve can experience more Alzheimer’s disease pathology in the brain (more plaques and tangles) without developing Alzheimer’s disease symptoms.

How does that work? Scientists are not sure but two possibilities are considered.
1. One is that more cognitive reserve means more brain reserve, that is more neurons and connections between neurons.
2. Another possibility is that more cognitive reserve means more compensatory processes (see my previous post “Education builds Cognitive Reserve for Alzheimers Disease Protection” for more details.)

Now, one may wonder about the difference types of mental stimulation available, including not only puzzles and such, but structured activities such as brain fitness software and meditation. Do we exercise our brain every time we think about something? What can one do to exercise one’s brain in ways that enhance capacity? Does aerobic fitness training also exercise one’s brain? What types of methodologies and products are available? Do they “work”? Are all the same?

Pascale MichelonPascale Michelon, Ph. D., is SharpBrains’ Research Manager for Educational Projects. Dr. Michelon has a Ph.D. in Cognitive Psychology and has worked as a Research Scientist at Washington University in Saint Louis, in the Psychology Department. MetLife Mature Market Institute Report: Gerontologist Fay Radding presents the findings of a recent MetLife report, concluding that “As individuals age, meaningful interactions and purposeful activity become even more valued and crucial to cognitive health- and cognitive health itself becomes more of a priority.”

Change Your Environment, Change Yourself: Dr. Brett Steenbarger explains in his recent book that, “The greatest enemy of change is routine. When we lapse into routine and operate on autopilot, we are no longer fully and actively conscious of what we’re doing and why. Michael Merzenich: Brain Plasticity offers Hope for Everyone

Recently there has been growing controversy about the effectiveness of computer-based cognitive training programs. As a co-founder of Posit Science, Inc. Dr. Michael Merzenich is a staunch defender of the methods his company uses to validate the programs that they have developed. But for the purposes of this essay, I want to share some of the key ideas we discussed during his recent interview on the Brain Science Podcast.

First of all, I asked him to discuss some of the highlights of his long career. Since he was one of the first neuroscientists to embrace the concepts of neuroplasticity I was curious about how this came about. Mind Games: the August issue of Venture Capital Journal brings a very good piece on the emerging brain fitness software category (subscription required), which we enhance by providing a quick overview of the field.

CogniFit raises USD 5 million: if 2007 was the year of brain fitness media coverage, 2008 seems to be the year of serious investments. This CogniFit round follows other recent venture investments: Dakim ($10.6m), Lumos Labs ($3m). We hear all these companies are devoting part of these resources to fund clinical trials…never too late.

Brain Failure and Brain Fitness: A Farewell to Dementia?

A dreaded diagnosis, that dimmed and dooming dilemma. Feared, sometimes fought, too often forgotten. It is the grayest, ghastliest elephant in the room: dementia.

What is dementia? I, like many others who dedicate their professional efforts to its study and treatment, have no good answer. I believe we are lost in our lexicon, trying to define a brain state so vexing and elusive it drives us out of our minds.

I hope we can do better, and I am not alone. In a sensitive and forward-looking editorial entitled Dementia: A Word to be Forgotten, Drs. Trachtenberg and Trojanowski of the University of Pennsylvania argue that alternate terms are more appropriate for research, clinical, and everyday settings. From scientific and biological perspectives, dementia is unspecific and subjective. Within the walls of the physician’s office, delivering the diagnosis of dementia can erect unintended walls around patients and families; vulnerable individuals, assuming that the “cruel connotations in the lay language” actually apply to them, are unnecessarily isolated. Minding the Aging Brain

It’s all in your head

The mind is not set in stone, but it is encased by bone. It’s really all about the brain, the hyphen in the mind-body conundrum. That squishy gray neuronal jungle is the interface between internal life and environmental sensations and stimulation. As expected, the brain shows signs of aging just as a wrinkled brow, a stooped posture, or an arthritic finger might. Cognitive Reserve and Intellectually Demanding Jobs

I hope you are having happy holidays, and are getting ready for New Year celebrations. Best wishes to you and your loved ones. 

Via MedJournalWatch we just found this interesting paper, 

Associations of job demands and intelligence with cognitive performance among men in late life. Guy G. Potter PhD*, Michael J. Helms BS, and Brenda L. Plassman PhD Neurology 2007.

- CONCLUSIONS: “Intellectually demanding work was associated with greater benefit to cognitive performance in later life independent of related factors like education and intelligence. The fact that individuals with lower intellectual aptitude demonstrated a stronger positive association between work and higher cognitive performance during retirement suggests that behavior may enhance intellectual reserve, perhaps even years after peak intellectual activity.”

This is consistent with the Cognitive Reserve theory we discussed in the interview with neuroscientist Yaakov Stern:

- AF (Alvaro Fernandez): OK, so our goal is to build that Reserve of neurons, synapses, and skills. How can we do that? What defines “mentally stimulating activities” or good “brain exercise”?

- YS (Yaakov Stern): In summary, we could say that “stimulation” consists of engaging in activities. In our research almost all activities are seen to contribute to reserve. Some have challenging levels of cognitive complexity, and some have interpersonal or physical demands. In animal studies, exposure to an enriched environment or increased physical activity result in increased neurogenesis (the creation of new neurons). You can get that stimulation through education and/ or your occupation. There is clear research showing how those two elements reduce the risk. Now, what is very exciting is that, no matter one’s age, education and occupation, our level of participation in leisure activities has a significant and cumulative effect. A key message here is that different activities have independent, synergistic, contributions, which means the more things you do and the earlier you start, the better. But you are never stuck: better late than never.

In short, mentally and socially stimulating activities, through our education, occupation AND leisure activities, contribute to building a Cognitive Reserve in our brains that may help delay memory problems, Mild Cognitive Impairment, and Alzheimer’s related symptoms, and help maintain cognitive performance overall as we age.
