Does Coffee Boost Brain/ Cognitive Functions Over Time?

A fewA_small_cup_of_coffee eternal questions:
- Is caffeine good for the brain?
- Does it boost cognitive functions?
- Does it protect against dementia?

There is little doubt that drinking that morning cup of coffee will likely increase alertness, but the main questions that research is trying to answer go beyond that. Basically: is there a sustained, lifetime, benefit or harm from drinking coffee regularly?

The answer, so far, contains good news and bad news. The good news for coffee drinkers is that most of the long-term results are directionally more positive than negative, so no clear harm seems to occur. The bad news is that it is not clear so far whether caffeine has beneficial effects on general brain functions, either short-term or long-term (aged-related decline or risks of dementia).

It is important to note that many of the studies showing an effect of coffee consumption on brain functions or risks of dementia report a correlation or association (they are not randomized clinical trials). As you know, correlation doesn’t prove causation: coffee drinkers may seem to do well in a number in these long-term studies, but there may be other reasons why coffee drinkers do better.

Q: How does caffeine affect my brain?
A: Caffeine is a stimulant.

It belongs to a chemical group called xanthine. Adenosine is a naturally occurring xanthine in the brain that slows down the activity of brain cells (neurons). To a neuron, caffeine looks like adenosine. It is therefore used by some neurons in place of adenosine. The result is that these neurons speed up instead of slowing down.

The Best Memory Tests: Mini-Mental and Beyond (Alzheimer’s Action Plan)

(Editor’s Note: I recently came across an excellent book and resource, The Alzheimer’s Alzheimer's Disease Action PlanAction Plan: The Experts’ Guide to the Best Diagnosis and Treatment for Memory Problems, recently released in paperback. Dr. Murali Doraiswamy, one of the authors and leading Alzheimer’s expert, kindly helped us create a 2-part article series to share with SharpBrains readers advice on a very important question, “How can we help the public at large to distinguish Alzheimer’s Disease from normal aging — so that an interest in early identification doesn’t translate into unneeded worries?” What follows is an excerpt from the book, pages 72-78, discussing the Pros and Cons of the most common assessments).

While no single test (other than a brain biopsy, which is a very invasive and risky procedure) can conclusively prove that a person has Alzheimer’s, many tests can give us a good idea. WHAT A DIFFERENCE AN EXTRA TEST CAN MAKE

Does cognitive training work? (For Whom? For What?)

The growing field of cognitive training (one of the tools for brain fitness) can appear very confusing as the media keeps reporting contradictory claims. These claims are often based on press releases, without a deeper evaluation of the scientific evidence.

Let’s take a couple of recent examples, in successive days:

“It doesn’t work!” type of headline:
Reuters (Feb. 10, 2009) — Formal brain exercise won’t help healthy seniors: research
“Healthy older people shouldn’t bother spending money on computer games and websites promising to ward off mental decline, the author of a review of scientific evidence for the benefits of these “brain exercise” programs says”.

“It works!” type of headline:
ScienceDaily (Feb. 11, 2009) — Computer Exercises Improve Memory And Attention, Study Suggests
“According to the researchers, participants who used the Brain Fitness Program also scored as well as those ten years younger, on average, on memory and attention tests for which they did not train.”

So… does structured brain exercise / cognitive training work or not?

The problem may in fact reside in asking this very question in the first place, as Alvaro pointed out a while ago in his article “Alzheimer’s Disease: too serious to play with headlines“.

We need a more nuanced set of questions.

Why? Because:
1. Cognition is made of several different abilities (working memory, attention, executive functions such as decision-making, etc)
2. Available training programs do not all train the same abilities
3. Users of training programs do not all have the same needs or goals
4. We need to differentiate between enhancing cognitive functions and delaying the onset of cognitive deficits such as Alzheimer’s.

Study Questions Effectiveness Of $80 Million Per Year ‘Brain Exercise Products Industry for Elderly (Science Daily)

- “There is much research on the benefits of cognitive rehabilitation strategies among elderly who already experience mild cognitive impairment (MCI) or Alzheimer’s disease, as well as on the positive impact of physical exercise. The researchers, however, wanted to evaluate current research that would focus on the impact of cognitive interventions in the healthy elderly population.”

- “…they concluded that there was no evidence indicating that structured cognitive intervention programs had an impact on the progression of dementia in the healthy elderly population”

“This report is interesting and it addresses many very important questions that cognitive neuropsychologists, such as myself have. I feel that many of the products on the market now make claims which are generally unsubstantiated.

I find it concerning that many of these programmes have been marketed to target older adults in particular without making any specific statement on whether the activities are beneficial and have been supported with empirical research.

Improve Memory and Enhance Post-Stroke Rehab with Exercise

A couple of recent studies have reinforced the lifelong potential for brain plasticity (the Brain Health Newsability of the brain to rewire itself through experience) and the importance of physical exercise for cognitive vitality. One study focused on 1) adults over 50 with mild cognitive impairment, the other one on 2) stroke survivors.

1)  Memory problems: Adults 50-years-old and over with mild cognitive impairment (an advanced form of memory problems, but pre-dementia) were asked to exercise for three 50-minute sessions per week for 24 weeks (a total of 60 hours). Report: The State of the Brain Fitness/ Training Software Market 2008

After many months of work (and we hope many new neurons and stronger synapses in our brains), we have just released our inaugural report on the emerging Brain Fitness Software Market, Brain Fitness Software the first to define the brain fitness and training software market and analyze the size and trends of its four customer segments. We estimate the size of the US brain fitness software market at $225M in2007, up from $100m in 2005 (50% CAGR). The two segments that fueled the market growth: consumers (grew from $5m to $80m, 300% CAGR) and healthcare & insurance providers (grew from $36m to $65m, 35% CAGR).

Highlights from The State of the Brain Fitness Software Market 2008 report include:

1) 2007 was a seminal year for the US Brain Fitness software market, which reached $225 million in revenues – up from an estimated $100 million in 2005.

2) Over 20 companies are offering tools to assess and train cognitive skills to four customer segments: consumers; healthcare and insurance providers; K12 school systems; and Fortune 1000 companies, the military, and sports teams.

Brain Training Games: Context, Trends, Questions

A spate of recent news coverage on brain fitness and “brain training” reflects a growing interest in natural, non-drug-based interventions to keep our brains sharp as we age. This interest is very timely, given the aging population, increasing Alzheimer’s rates, and soaring health care costs that place more emphasis than ever on prevention and changing lifestyle.

This past Tuesday, the MIT Club of Northern California, the American Society on Aging, and SmartSilvers sponsored an event on The Emerging Brain Fitness Software Market: Building Better Brains to explore the realities and myths of this growing field. The panel was moderated by Zack Lynch, Executive Director of the Neurotechnology Industry Organization, and composed of a venture capitalist and 3 CEOs of program developers in the field. Before the panel, I had the chance to present an overview of the state of the Brain Fitness Software Market based on our upcoming report to be released on March 4th.

Why are we talking about this field at all? Well, for one, an increasing number of companies are achieving significant commercial success in packaging “brain exercise”. Fish Oil May Help Prevent Alzheimer’s (Washington Post)

- “The omega-3 fatty acids found in fish oil might play an important role in preventing Alzheimer’s disease, according to a research team at the University of California, Los Angeles (UCLA).”

- “Publishing in the Dec. 26 issue of the Journal of Neuroscience, the scientists demonstrated that the omega-3 fatty acid docosahexaenoic acid (DHA) increases the production of LR11, a protein that is found at reduced levels in Alzheimer’s patients. LR11 is known to destroy the protein that forms the plaques associated with the disease, the researchers explained.” 

- “Alzheimer’s is a debilitating neurodegenerative disease that causes memory loss, dementia, personality change and ultimately death. Cognitive Reserve and Intellectually Demanding Jobs

I hope you are having happy holidays, and are getting ready for New Year celebrations. Best wishes to you and your loved ones. 

Via MedJournalWatch we just found this interesting paper, 

Associations of job demands and intelligence with cognitive performance among men in late life. Guy G. Potter PhD*, Michael J. Helms BS, and Brenda L. Plassman PhD Neurology 2007.

- CONCLUSIONS: “Intellectually demanding work was associated with greater benefit to cognitive performance in later life independent of related factors like education and intelligence. The fact that individuals with lower intellectual aptitude demonstrated a stronger positive association between work and higher cognitive performance during retirement suggests that behavior may enhance intellectual reserve, perhaps even years after peak intellectual activity.”

This is consistent with the Cognitive Reserve theory we discussed in the interview with neuroscientist Yaakov Stern:

- AF (Alvaro Fernandez): OK, so our goal is to build that Reserve of neurons, synapses, and skills. How can we do that? What defines “mentally stimulating activities” or good “brain exercise”?

- YS (Yaakov Stern): In summary, we could say that “stimulation” consists of engaging in activities. In our research almost all activities are seen to contribute to reserve. Some have challenging levels of cognitive complexity, and some have interpersonal or physical demands. In animal studies, exposure to an enriched environment or increased physical activity result in increased neurogenesis (the creation of new neurons). You can get that stimulation through education and/ or your occupation. There is clear research showing how those two elements reduce the risk. Now, what is very exciting is that, no matter one’s age, education and occupation, our level of participation in leisure activities has a significant and cumulative effect. A key message here is that different activities have independent, synergistic, contributions, which means the more things you do and the earlier you start, the better. But you are never stuck: better late than never.

In short, mentally and socially stimulating activities, through our education, occupation AND leisure activities, contribute to building a Cognitive Reserve in our brains that may help delay memory problems, Mild Cognitive Impairment, and Alzheimer’s related symptoms, and help maintain cognitive performance overall as we age.
