Alzheimer’s Early and Accurate Diagnosis: Normal Aging vs. Alzheimer’s Disease

(Editor’s Note: I recently came across an excellent book and resource, The Alzheimer’s Alzheimer's Disease Action PlanAction Plan: The Experts’ Guide to the Best Diagnosis and Treatment for Memory Problems, just released in paperback. Dr. Murali Doraiswamy, one of the authors and leading Alzheimer’s expert, kindly helped us create a 2-part article series to share with SharpBrains readers advice on a very important question, “How can we help the public at large to distinguish Alzheimer’s Disease from normal aging — so that an interest in early identification doesn’t translate into unneeded worries?” What follows is an excerpt from the book, pages 3-8).

Jane, fifty-seven, managed a large sales force. She prided herself on being good at names, and introductions were easy for her—until last spring when she referred to Barbara as Betty at a meeting and had to correct herself. She started noticing that her memory wasn’t as dependable as it once was—she had to really try to remember names and dates. Her mother had developed Alzheimer’s in her late seventies, so Jane entertained a wide array of worries: Is this just aging? Is it because of menopause? Is it early Alzheimer’s? Did her coworkers or family notice her slips? Should she ask them? Should she see a doctor, and if so, which doctor? Would she really want to know if she was getting Alzheimer’s? Would she lose her job, health insurance, or friends if she did have Alzheimer’s?

As it turns out, Jane did not have Alzheimer’s. She consulted a doctor, who, in docspeak, told her that the passage of time (getting older) had taken a slight toll on her once-superquick memory. She was slowing down a little, and if she relaxed, the name or date or other bit of information she needed would come to her soon enough. She was still good at her job and home life. She had simply joined the ranks of the worried well.

Normal brain aging, beginning as early as the forties in some people, may include:

• Taking longer to learn or remember information
• Having difficulty paying attention or concentrating in the midst of distractions
• Forgetting such basics as an anniversary or the names of friends
• Needing more reminders or memory cues, such as prominent appointment calendars, reminder notes, a phone with a wellstocked speed dial

Although they may need some assistance, older people without a mental disorder retain their ability to do their errands, handle money, find their way to familiar areas, and behave appropriately.

Are videogames good for your brain?: A landmark study by Dr. Arthur Kramer and colleagues has shown that playing a strategy videogame can bring a variety of significant mental benefits to older brains. Another recent study, also by Kramer and colleagues, does not show similar benefits to younger brains (despite playing the same game). How can this be? Dr. Kramer, who has kindly agreed to serve on SharpBrains’ Scientific Advisory Board, elaborates.

Meditation on the Brain: Dr. Andrew Newberg provides an excellent overview of the brain benefits of practices such as meditation. He recommends, “look for something simple, easy to try first, ensuring the practice is compatible with one’s beliefs and goals. You need to match practice with need: understand the specific goals you have in mind, your schedule and lifestyle, and find something practical.”

The Need for Objective Assessments

Cognitive screenings and Alzheimer’s Disease: The Alzheimer’s Foundation of America just released a thoughtful report advocating for widespread cognitive screenings after the age of 65 (55 given the right conditions). SharpBrains readers, probed by Dr. Joshua Steinerman, seem to agree.

Quantitative EEG for ADHD diagnosis: Dr. David Rabiner reports on the findings from a recent study that documents the utility of Quantitative EEG as an objective test to assist in the diagnosis of ADHD. If this procedure were to become more widely used, he suggests, the number of children and adolescents who are inappropriately diagnosed and treated for the disorder would diminish substantially.

Shall we question the brand new book of human troubles?: The fights over the new version of the psychiatric diagnostic manual, the DSM-V, are starting to come to light. Dr. Vaughan Bell wonders why the public debate avoids the key question of whether diagnosis itself is useful for mental health and why psychometrics are simply ignored.

Education builds Cognitive Reserve for Alzheimers Disease Protection: Dr. Pascale Michelon reviews a recent study that supports the Cognitive Reserve hypothesis – mentally stimulating experiences throughout life, such as formal education, help build a reserve in our brains that contributes to a lower probability of developing Alzheimer’s symptoms.

5 Tips on Lifelong Learning & the Adult Brain: Laurie Bartels asks us to please please 1) challenge ourselves with new learning, 2) remember that neuroplasticity and neurogenesis are hallmarks of our brains, 3) check for mis-learning on an ongoing basis, 4) more visuals, less text, 5) move it, move it – start today!

Neuroscience Core Concepts: We all have heard “Use It or Lose It”. Now, what is “It”? The Society for Neuroscience (SfN) has just released a user-friendly publication titled Neuroscience Core Concepts, aimed at helping educators and the general public learn more about the brain.

Brain Health: Physical or Mental Exercise?

Our fellow blogger Jeremy over at PsyBlog has written a thoughtful post comparing the value of a number of cognitive enhancing tools. His overall verdict?

The evidence for exercise boosting cognitive function is head-and-shoulders above that for brain training, drugs, nutritional supplements and meditation. Scientifically, on the current evidence, exercise is the best way to enhance your cognitive function. And as for its side-effects: yes there is the chance of an injury but exercise can also reduce weight, lower the chance of dementia, improve mood and lead to a longer life-span. Damn those side-effects!”

Article: Which Cognitive Enhancers Really Work: Brain Training, Drugs, Vitamins, Meditation or Exercise?

Jeremy, I started writing this as a comment to your post in your blog, but then it got too long. Let me write my reaction to your post here. 

While I appreciate your analysis and share most of your points, I think the “ranking” effort (this type of intervention is better than that one) is ultimately misleading.  It is Rubik's Cube brain exercisebased on a faulty search for a general solution/ magic pill for everyone and everything.

If only things were so simple. Perhaps one day there will be research to support that view, but certainly not today. A number of interventions have shown their value. In different populations, and contexts. For “exercise is the best way to enhance your cognitive function” to be true, one needs to have a pretty specific understanding of “best”, “your” and “cognitive function”.

Brain Training Games: Context, Trends, Questions

A spate of recent news coverage on brain fitness and “brain training” reflects a growing interest in natural, non-drug-based interventions to keep our brains sharp as we age. This interest is very timely, given the aging population, increasing Alzheimer’s rates, and soaring health care costs that place more emphasis than ever on prevention and changing lifestyle.

This past Tuesday, the MIT Club of Northern California, the American Society on Aging, and SmartSilvers sponsored an event on The Emerging Brain Fitness Software Market: Building Better Brains to explore the realities and myths of this growing field. The panel was moderated by Zack Lynch, Executive Director of the Neurotechnology Industry Organization, and composed of a venture capitalist and 3 CEOs of program developers in the field. Before the panel, I had the chance to present an overview of the state of the Brain Fitness Software Market based on our upcoming report to be released on March 4th.

Why are we talking about this field at all? Well, for one, an increasing number of companies are achieving significant commercial success in packaging “brain exercise”. An example is the line of Nintendo games, such as Brain Age and Brain Training, that have shipped over 15 million units worldwide despite limited scientific support, since 2005. Cognitive Reserve and Intellectually Demanding Jobs

I hope you are having happy holidays, and are getting ready for New Year celebrations. Best wishes to you and your loved ones. 

Via MedJournalWatch we just found this interesting paper, 

Associations of job demands and intelligence with cognitive performance among men in late life. Guy G. Potter PhD*, Michael J. Helms BS, and Brenda L. Plassman PhD Neurology 2007.

- CONCLUSIONS: “Intellectually demanding work was associated with greater benefit to cognitive performance in later life independent of related factors like education and intelligence. The fact that individuals with lower intellectual aptitude demonstrated a stronger positive association between work and higher cognitive performance during retirement suggests that behavior may enhance intellectual reserve, perhaps even years after peak intellectual activity.”

This is consistent with the Cognitive Reserve theory we discussed in the interview with neuroscientist Yaakov Stern:

- AF (Alvaro Fernandez): OK, so our goal is to build that Reserve of neurons, synapses, and skills. How can we do that? What defines “mentally stimulating activities” or good “brain exercise”?

- YS (Yaakov Stern): In summary, we could say that “stimulation” consists of engaging in activities. In our research almost all activities are seen to contribute to reserve. Some have challenging levels of cognitive complexity, and some have interpersonal or physical demands. In animal studies, exposure to an enriched environment or increased physical activity result in increased neurogenesis (the creation of new neurons). You can get that stimulation through education and/ or your occupation. There is clear research showing how those two elements reduce the risk. Now, what is very exciting is that, no matter one’s age, education and occupation, our level of participation in leisure activities has a significant and cumulative effect. A key message here is that different activities have independent, synergistic, contributions, which means the more things you do and the earlier you start, the better. But you are never stuck: better late than never.

In short, mentally and socially stimulating activities, through our education, occupation AND leisure activities, contribute to building a Cognitive Reserve in our brains that may help delay memory problems, Mild Cognitive Impairment, and Alzheimer’s related symptoms, and help maintain cognitive performance overall as we age.
