The Age of Mass Intelligence?

We’ve all heard about dumb­ing down. But there is plenty of evi­dence that the oppo­site is also true. Is this, in fact, the age of mass intelligence?”

Fas­ci­nat­ing arti­cle in Intel­li­gent Life, a pub­li­ca­tion by The Economist:

THE AGE OF MASS INTELLIGENCE

- “Mil­lions more peo­ple are going to muse­ums, lit­er­ary fes­ti­vals and operas; mil­lions more watch demand­ing tele­vi­sion pro­grammes or down­load serious-minded pod­casts. Not all these activ­i­ties count as mind-stretching, of course. Some are down­right fluffy. But, says Donna Ren­ney, the chief exec­u­tive of the Chel­tenham Fes­ti­vals, audi­ences increas­ingly want “the buzz you get from work­ing that lit­tle bit harder”. This is a dra­matic yet often unrecog­nised devel­op­ment. “When peo­ple talk and write about cul­ture,” says Ira Glass, the cre­ator of the riv­et­ing public-radio show “This Amer­i­can Life”, “it’s apoc­a­lyp­tic. We tell our­selves that every­thing is in bad shape. But the oppo­site is true. There’s an abun­dance of really inter­est­ing things going on all around us.”

- “Third, what does all this say about the wide­spread view that soci­eties are dumb­ing down, edu­ca­tional stan­dards are crum­bling and people’s abil­ity to con­cen­trate is col­laps­ing? The reply must be that it can­not be true across the board and that for a sig­nif­i­cant num­ber, the oppo­site is the case: peo­ple want more intel­lec­tu­ally demand­ing things to see and hear, not fewer. Surely both things are hap­pen­ing at once: part of the pop­u­la­tion is dumb­ing down, part is wis­ing up.”

Education builds Cognitive Reserve for Alzheimers Disease Protection

Given the grow­ing media cov­er­age men­tion­ing the terms Cog­ni­tive Reserve and Brain Reserve, you may be ask­ing your­self, “What exactly is my Cog­ni­tive (or Brain) Reserve?”

The cog­ni­tive reserve hypoth­e­sis, tested in mul­ti­ple stud­ies, states that indi­vid­u­als with more cog­ni­tive reserve can expe­ri­ence more Alzheimer’s dis­ease pathol­ogy in the brain (more plaques and tan­gles) with­out devel­op­ing Alzheimer’s dis­ease symptoms.

How does that work? Sci­en­tists are not sure but two pos­si­bil­i­ties are con­sid­ered.
1. One is that more cog­ni­tive reserve means more brain reserve, that is more neu­rons and con­nec­tions (synapses) between neu­rons. Indi­vid­u­als with more synapses would then have more synapses to lose before the crit­i­cal thresh­old for Alzheimer’s Dis­ease is reached.
2. Another pos­si­bil­ity is that more cog­ni­tive reserve means more com­pen­satory processes. The brain of indi­vid­u­als with more cog­ni­tive reserve would use more alter­na­tive net­works to com­pen­sate for the dam­ages caused by the pathol­ogy in pre­vi­ously used networks.

In a newly pub­lished study, Roe and col­leagues brain fitness event from Wash­ing­ton Uni­ver­sity in St. Louis, used the num­ber of years of edu­ca­tion as a mea­sure of cog­ni­tive reserve. Why years of edu­ca­tion? Because pre­vi­ous stud­ies have shown that peo­ple who have more edu­ca­tion also exhibit a greater resis­tance to Alzheimer’s symp­toms, even while patho­log­i­cal changes are occur­ring in the brain (see Ben­nett el al., 2003 or Roe, Xiong, et al., 2008).

Roe and her col­leagues stud­ied 198 indi­vid­u­als whose mean age was 67. Out of these 198 indi­vid­u­als, 161 were non­de­mented and 37 were diag­nosed with Alzheimer’s Disease.

Games for Brain Health — Novelty, Variety and Challenge

Land­mark study just pub­lished: Basak C, et al “Can train­ing in a real-time strat­egy video game atten­u­ate cog­ni­tive decline in older adults?” Psy­chol Aging 2008; DOI: 10.1037/a0013494.

Play­ing com­puter games improves brain power of older adults, claim sci­en­tists (Telegraph)

- The team at the Uni­ver­sity of Illi­nois recruited 40 adults over 60 years old, half of whom were asked to play a com­puter game called Rise of Nations, a role-playing game in which you have to build your own empire.

- Game play­ers have to build cities, feed and employ their peo­ple, main­tain an ade­quate mil­i­tary and expand their territory.

- Both groups were assessed before, dur­ing and after the video game train­ing on a vari­ety of tests.

- As a group, the “gamers” became sig­nif­i­cantly bet­ter – and faster – at switch­ing between tasks as com­pared to the com­par­i­son group. Their work­ing mem­ory, as reflected in the tests, was also sig­nif­i­cantly improved and their rea­son­ing abil­ity was enhanced.

- (Pro­fes­sor Art Kramer, an author of the study pub­lished in the jour­nal Psy­chol­ogy & Aging) “This is one mode in which older peo­ple can stay men­tally fit, cog­ni­tively fit. I’m not sug­gest­ing, how­ever, that it’s the only thing they should do.”

Pro­fes­sor Kramer and I dis­cussed this study last June dur­ing our con­ver­sa­tion on Why We Need Walk­ing Book Clubs:

The reporter and I also dis­cussed in depth the need for bet­ter con­sumer edu­ca­tion and pro­fes­sional devel­op­ment, so peo­ple can make informed deci­sions, and for cog­ni­tive assess­ments to serve as inde­pen­dent base­line, help iden­tify pri­or­i­ties and mea­sure results. Please note that our mar­ket esti­mates do include rev­enues of com­put­er­ized cog­ni­tive assess­ments, today mostly used in clin­i­cal tri­als, and within the mil­i­tary and sports teams.

Cognitive screenings and Alzheimer’s Disease

The Alzheimer’s Foun­da­tion of Amer­ica just released a thought­ful report advo­cat­ing for wide­spread cog­ni­tive screen­ings after the age of 65 (55 given the right conditions).

Accord­ing to the press release,

- “The report shat­ters unsub­stan­ti­ated crit­i­cism and instead empha­sizes the safety and cost-effectiveness of these tools and calls on Con­gress to develop a national demen­tia screen­ing policy.”

- “Lift­ing the bar­ri­ers to early detec­tion is long over­due, Hall said. “Con­ver­sa­tions about brain health are not tak­ing place. We must edu­cate and empower con­sumers to talk openly about mem­ory con­cerns, par­tic­u­larly with pri­mary care providers, so they get the atten­tion and qual­ity of life they deserve.

- “Demand for screen­ings is evi­denced by the suc­cess of AFA’s recent sixth annual National Mem­ory Screen­ing Day held on Novem­ber 18, dur­ing which an esti­mated 50,000 peo­ple were given free con­fi­den­tial mem­ory screen­ings at nearly 2,200 com­mu­nity sites nation­wide. Dur­ing last year’s event, approx­i­mately 16 per­cent of indi­vid­u­als who had a face-to-face screen­ing scored pos­i­tive and were referred to their pri­mary care providers for follow-up. An AFA sur­vey of par­tic­i­pants revealed that fewer than one in four with self-reported mem­ory com­plaints had pre­vi­ously dis­cussed them with their physi­cians despite recent visits.”

Please note that the Alzheimer’s Asso­ci­a­tion recently argued in the oppo­site direc­tion (no screen­ings) — which prob­a­bly trig­gered this response.

We see emerg­ing trends that sug­gest the posi­tion in favor of cog­ni­tive assess­ments may in fact gather momen­tum over the next few years: wide­spread com­put­er­ized cog­ni­tive screen­ings in the US Army, insur­ance com­pa­nies like OptumHealth adding such tools to its clin­i­cal decision-making sys­tems, polls such as the Amer­i­can Soci­ety of Aging’s a cou­ple of years ago indi­cat­ing a very strong demand for an “annual men­tal check-up”, the avail­abil­ity of use­ful assess­ment tools and research-based pre­ven­tive advice.

The start­ing point is to under­stand what those assess­ments are NOT: they are not diag­nos­tic tools. When used prop­erly, they can be used as a base­line to track per­for­mance in a vari­ety of cog­ni­tive domains over time, so that both the indi­vid­ual AND the physi­cian are not blinded by a one-time assess­ment, com­par­ing an indi­vid­ual with his or her peers (instead of his or her past per­for­mance) when seri­ous symp­toms have fre­quently already been going on for a while.

So did President-elect Obama came to real­ize a few days ago. After his peo­ple kindly con­tacted our peo­ple, we felt com­pelled to grant him open access to our col­lec­tive wis­dom. With­out fur­ther ado, below you have Grand Rounds 5:12 — a Q&A ses­sion led by the incom­ing Pres­i­dent on how to reform (for the bet­ter, we hope) healthcare.

On Health Insurance

Q:  How does the blo­gos­phere per­ceive the prob­lem of hav­ing a sig­nif­i­cant group of peo­ple uninsured?

Health Insur­ance Col­orado: a grow­ing eco­nomic bur­den, which may lead to emer­gency rooms turn­ing peo­ple away if they are unable to pro­vide proof of health insurance.

Dr Rich: well, a recent arti­cle in the Jour­nal of the Amer­i­can Med­ical Asso­ci­a­tion showed how over­crowd­ing in Amer­i­can emer­gency rooms is NOT due to the unin­sured. Rather, it is due to insured Amer­i­cans who can­not get in to see their pri­mary care physi­cians. We may need improved care both for the insured and unin­sured groups.

Insure­Blog: I’d sec­ond that. Lack of health insur­ance is a major prob­lem but is it really our Biggest Problem?
